Nurse Practitioners

The Nurse Practitioner is an integral part of the team who will collaborate with the family physicians and other interdisciplinary providers to achieve the vision and mission of the organization. The Nurse Practitioner will help the Family Health Team to meet its strategic objectives by improving patient access to care, participating in health promotion and disease prevention, and treating and educating patients with chronic disease issues.
